Worksheets for ETOS V5B

This document, labeled as Appendix A, provides blank worksheets to be used to record the current values of various options selected under the ETOS V5B system. It includes tables (Table A-1 and A-2) that list program file names, group names, decimal lengths, and space to record write lengths. Users are instructed to make copies of these worksheets, fill them out as the system is built, record changes in pencil, and keep them with their Manager's Guide.
